As the Clinical Care Coordinator, you’ll take the lead in overseeing and coordinating high-quality clinical care. With a dedicated team behind you, supported by a committed Care Manager and Facility Manager, you’ll help elevate care standards for residents and staff alike.
Key Responsibilities:
- Lead and support a team of nursing professionals, providing coaching and mentoring.
- Collaborate with healthcare teams to develop and implement effective care plans.
- Ensure compliance with documentation and clinical governance standards.
- Oversee infection control protocols and medication management.
- Build and nurture strong relationships with residents, families, and stakeholders to promote optimal health outcomes.
What We’re Looking For:
- Tertiary qualification in Nursing with AHPRA Registration.
- Strong knowledge of aged care regulatory requirements, clinical governance, and clinical standards.
- Full Australian working rights
- Proven leadership experience in managing teams and complex care scenarios.
- Understanding of AN-ACC assessment processes.
- Previous experience in a similar role within a Residential Aged Care setting
